Consent Center
Two instruments. Two signatures. Never bundled.
Treatment consent is required to receive work. Media authorization is optional and is never a condition of care, price, or availability. They are signed separately here, and each one is filed on its own.
Signing here requires that you are 18 or older. A guardian path for minors is not yet open; a session for someone under 18 must be arranged with the practice directly.
Every signature produces a sealed copy with its own audit ID and content hash. You can open your copy at the end of this page.
One — Who is signing

Two — Required
Treatment Consent v1.0
What this work is
Silentium provides therapeutic massage and restorative bodywork. This is not medical care. Nothing performed here is a diagnosis, a prescription, or treatment of a medical condition, and it does not replace care from a physician or other licensed provider.
What you agree to tell me
You confirm that the intake information you have given is accurate and complete to the best of your knowledge, including injuries, recent surgery, pregnancy, skin conditions, medications, and anything that changes how the body should be worked. If something changes before or during the session, you will say so.
Draping and boundaries
You remain draped throughout. Only the area being worked is uncovered. Breast, genital, and gluteal cleft tissue is never worked. Work is performed with your clothing choice respected and your stated boundaries honored without discussion or persuasion.
You may stop at any time
You may ask for less pressure, a different area, a pause, or an end to the session at any point, for any reason or none. Ending a session early is not a problem and carries no penalty beyond the practice's stated cancellation terms.
Reasonable response
Bodywork can produce temporary soreness, bruising, lightheadedness, or emotional release. Cupping commonly leaves temporary marking. These are ordinary responses, not injuries, and they are discussed before any technique that causes them is used.
Records
Silentium keeps a client record and a session record, including intake information, practitioner notes, and preferences relevant to your care. These are retained and disposed of as described in the Privacy Notice.
Your signature
You have read this consent, you have had the opportunity to ask questions, and you consent to receive massage therapy at Silentium. This consent covers treatment only. It does not authorize any use of your image, voice, words, or name.
Sign above with a finger, stylus or mouse.
Three — Optional
Media Authorization v1.0
Anything I do not select is not authorized. Silentium may not treat silence, prior participation, or a previous authorization as permission for a use I have not selected.
Optional, always
This authorization is optional. It is never a condition of care, price, access, availability, or the quality of the work you receive. Declining everything on this page changes nothing about your session.
Granted only by selection
Anything I do not select is not authorized. Silentium may not treat silence, prior participation, or a previous authorization as permission for a use I have not selected.
Withdrawal
You may withdraw any permission at any time through the Consent Center or by contacting Silentium. Withdrawal takes effect prospectively: material already published in reliance on a permission may not be recoverable, but no further use is made after withdrawal is recorded.
Separate signature
This authorization carries its own signature. It is never covered by your treatment consent, even when both are signed in the same sitting.
Sign above with a finger, stylus or mouse.
You may leave this unsigned. Declining changes nothing about your session.

Privacy Notice v1.0
What is collected
Your name and contact details, booking and payment records, intake information, Ritual Composer selections, practitioner notes about the work performed, and the consent records described below.
How it is used and shared
Silentium does not sell client information. Information is shared only as needed to operate the service, process bookings and payments, maintain records, comply with law, or when you specifically authorize another use.
How long it is kept
Client and business records are retained for at least three years from the date of service, or longer when another legal or operational requirement applies, and are disposed of securely.
Access and correction
You may contact Silentium to request access to, or correction of, information we maintain about you. Requests involving records that must be retained will be handled consistent with applicable law and professional recordkeeping requirements.
Ritual Composer
Ritual Composer selections may be retained with your session record when they are relevant to preparation, preferences, boundaries, or continuity of care.
Media
No image, video, audio, testimonial, name, or social identity is used for any purpose without a separate, signed Media Authorization naming that specific use.
